Family-Based Services for Adults with Intellectual or Developmental Disabilities
At the heart of the Family-Based program is Omni Visions’ belief that most individuals can best be served in a family setting. Historically, adults with developmental or intellectual disabilities often lived at home until family members could no longer care for them. Unfortunately, most then transitioned to large institutions or nursing homes. Through Omni's Family-Based services program, many of these citizens can live successfully in the community and families now have a family-based alternative to institutional living.
Services are provided through a network for trained Family-Based Providers. These individuals provide housing, supervision, support and personal care to the client or clients they serve. A Service Coordinator supports every family-based provider. These Service Coordinators partner with the family to arrange support services and are on call and available 24 hours a day, 7 days a week, when assistance, support, direction, and advocacy are needed.
